Shot requirements for 2008-2009

Dear Parents or Guardians of students from James Monroe Middle School,

All 6th grade students will need to be in compliance with the Department of Health vaccine requirements for 7th grade enrollment.  The required shots are Hepatitis B (series of 2 to 3 shots) and Tdap. The Tdap is required 5 years after the last DPT; usually last given around 4 years of age. 

All 8th grade students need to review their shot records for the last DPT they received. 8th grade students will need to have Tdap or its equivalent 5 years after the last DPT on their shot record. Bring all shots received to the health office to update the computer vaccine program.

Please have your student vaccinated this school year to avoid the long lines at registration.

Please call the health office to verify any shots you have concerns with.

If your student receives any vaccine during the summer you will need to bring the vaccine record to registration and wait in line to be processed for compliancy from the Department of Health requirements.

All APS schools are required to follow the Department of Health law on vaccines for children.

 Vaccines can be complicated, the health office is more than happy to assist with any issues.
